ABSTRACT

Purpose Our study aims to compile data on the clinical presentation, pathological and radiological findings in cases of post-COVID mucormycosis, and present the management strategy used in our center. Methods This is a retrospective cohort observational study based at a tertiary healthcare institution in Northern India. All COVID-positive patients presenting with clinical features of mucormycosis were included in the study. They underwent complete otorhinolaryngeal, medical, and ophthalmological examination after thorough history taking. Biochemical tests, biopsy and imaging studies were done for all the patients. The treatment strategy included a multidisciplinary team approach, that is, intravenous antifungals as well as surgical debridement of necrotic tissue via Modified Denker's approach or open maxillectomy, and orbital exenteration, if required. Patients were followed up for six months to look for recurrence. Results Twenty-three patients were studied, out of which 14 were males and 9 were females. Pathological findings of 13 out of 15 patients, who underwent surgical debridement revealed mucormycosis as a causative agent, received Amphotericin. Aspergillus was found in two cases which received Voriconazole. Eleven out of 20 patients who were treated in our hospital survived. Three patients were lost to follow up. The average hospital stay of discharged patients was 14 days. Conclusion Post-COVID mucormycosis was reported at an alarming rate after the second COVID wave in India especially after steroid therapies in diabetic patients. Thus a timely, aggressive, team approach using Modified Denkers or open maxillectomy along with proper intravenous antifungals is the key to survival in such patients.

ABSTRACT

Introduction: Zygomycetes consisting of Mucorales order is a group of fungal infections. These species cause life threatening opportunistic fungal infections mucormycosis. This infection is highly prevalent in immunocompromised. During the 2nd wave of Covid 19 pandemic corticosteroid treatment was used which has been linked to development of Mucormycosis. In our tertiary care teaching hospital we saw that patients suffering from Covid-19 infections developed mucormycosis. We present these cases in our study. To study the clinical, demographical,and Laboratory parameters in Covid-19 patients with Mucormycosis. Material and Methods: Retrospective Study. All biopsy proven cases of Mucormycosis (which developed after Covid-19 infection) were included. Relevant Clinical Demographics and Laboratory data was retrieved from the available case sheets. The data was tabulated in Excel sheet and further reviewed. Results: A total of 22 patients were diagnosed as suffering from mucormycosis majority were unvaccinated. 11 patients out of 22 (50%) started manifesting mucormycosis within one week of COVID infection. All the patients who had only single comorbidity (22.72%) suffered from mild disease and patient who had more than one comorbidity suffered from moderate (27.27%) to severe (50%) COVID infection. Conclusion: It is suggested that patients with Covid-19 infection are at risk for development of opportunistic fungal infections like Mucormycosis. Hence the physicians who are involved in treating such patients must be mindful of the fact that mucormycosis can develop in them. Histopathology helps in establishing a concrete diagnosis of Mucormycosis.

ABSTRACT

In today's information age, both excess and lack of information can cause a disaster. COVID-19 pandemic not only highlighted the significance of risk communication but also pointed out several unintended and distressing consequences due to information gaps and miscommunications. Despite facing a common threat, the local communities suffered differential impacts during the pandemic. This paper classifies the nature of risk communications experienced across different countries into three categories, namely: inadequate, ideal, and infodemic risk communication that influenced the local perceptions and responses. It further argues that inadequately planned risk communications tend to create new risks and compromise the efforts towards managing a disaster. As global risks are responded locally, there is a need for more inclusive and engaging risk communication that involves communities as responsible stakeholders who understand, plan, and respond to risks to increase their propensity for resilience during disasters and crisis situations.

ABSTRACT

OBJECTIVE: To evaluate the effect of folate and vitamin B12 levels on pregnancy progression and outcomes. METHODS: The present study is a prospective follow up study of 100 pregnant women. Biochemical investigations (plasma homocysteine, folate, and vitamin B12 levels) were performed on all pregnant women in first, second, and third trimesters. Nonparametric tests were used to compare the differences in median levels and odds ratio analysis for the assessment of the risk between the selected biomarkers and adverse pregnancy progression and outcomes. RESULTS: The pregnant women at their first antenatal care visit were found to be predominantly folate replete (97%) and vitamin B12 deficient (60%). Hyperhomocysteinemia in first and second trimesters was found to pose more than 3-fold increased risk for adverse pregnancy outcomes (P = .006 and .0002, respectively). Low birth weight (LBW) was found to be the most common adverse pregnancy outcome (52%), and was significantly associated with vitamin B12 deficiency in the first and second trimesters (82%, P < .0001; 71.4%, P = .04, respectively). CONCLUSION: The vitamin B12 deficiency is more common among Indian pregnant women as compared to folate deficiency. Hyperhomocysteinemia is an independent risk factor for pregnancy complications. Vitamin B12 deficiency in first and second trimesters is associated with LBW babies.

ABSTRACT

Mathematical models are mainly used to depict real world problems that humans encounter in their daily explorations, investigations and activities. However, these mathematical models have some limitations as indeed the big challenges are the conversion of observations into mathematical formulations. If this conversion is inefficient, then mathematical models will provide some predictions with deficiencies. A specific real-world problem could then have more than one mathematical model, each model with its advantages and disadvantages. In the last months, the spread of covid-19 among humans have become fatal, destructive and have paralyzed activities across the globe. The lockdown regulations and many other measures have been put in place with the hope to stop the spread of this deathly disease that have taken several souls around the globe. Nevertheless, to predict the future behavior of the spread, humans rely on mathematical models and their simulations. While many models, have been suggested, it is important to point out that all of them have limitations therefore newer models can still be suggested. In this paper, we examine an alternative model depicting the spread behavior of covid-19 among humans. Different differential and integral operators are used to get different scenarios.
